What this website collects

  • Information you type into a form on this site: your name, phone, email, the clinic you chose, what is bothering you, and for a benefits check your date of birth, ZIP, insurer and member ID
  • Whether you consented to receive text messages
  • Standard analytics about pages viewed, collected in aggregate

How we use it

To contact you about the appointment or benefits check you asked for, and to verify your coverage. We do not sell your information. We do not share it for advertising.

Text messages

If you check the SMS consent box, we may text you about your appointment. Message and data rates may apply. Reply STOP to opt out at any time. Consent to texts is never a condition of treatment.

Your health records

Your clinical record is protected health information under HIPAA. You have the right to see it, to request a correction, and to know who it has been disclosed to. Ask any front desk and we will start that request.
